Purulent Pericarditis Presenting as Cardiac Tamponade

Abstract

Purulent pericarditis is an uncommon, localized infection of the pericardial space. It can manifest as cardiac tamponade and must be considered in the differential diagnosis in patients with shock and chest pain.

Infection by Streptococcus agalactiae has been identified as an increasingly common cause of invasive disease in adults, especially amongst elderly and diabetic patients. It can have a variable presentation, including endocarditis and purulent pericarditis.

We report the case of a 70-year-old man with type 2 diabetes, who presented with purulent pericarditis manifested as cardiac tamponade and aortic valve endocarditis. The underlying mechanism was hematogenic spread of Streptococcus agalactiae after bacterial translocation.

This clinical case presents a serious and uncommon form of bacterial pericarditis, that must be remembered, and reinforces the usefulness of an early echocardiogram in patients with shock since it can be diagnostic, hasten the treatment and improve the prognosis of these patients.
